An in-depth “Deep Dive” health history and health assessment visit (Approximately 90 minutes)
We will determine which goals you would to like to set for yourself and develop an individualized success plan to help you reach them. Individualized printed resources will be provided
We will have five one on one sessions (approximately 45 minutes each)
At each visit we will reassess your progress towards your goals, adjust the plan as needed, and you will receive individualized printed resources as needed
You will have a weekly email, text or phone follow up
Unlimited email and text support to address any questions, concerns or problems you may be having
3 Month Reclaim Your Health And Change Your Life Package

We will have 4 one-on-one sessions each month - 12 sessions total (approximately 45 minutes each)
At each visit we will reassess your progress towards your goals, adjust the plan as needed, and you will receive individualized printed resources as needed
You will have a weekly email, text or phone follow up
Unlimited email and text support to address any questions, concerns or problems you may be having
Per Session Rates
I do offer per session rates for those who have a short term issue they need immediate assistance with, such as COVID prevention and early treatment protocol questions, those with medication questions or issues, those who need help advocating for themselves or their family members with physicians or healthcare institutions.
I also provide concierge nurse visits for all your personal nursing needs for local clients.
These visits will be billed as individual sessions.
